Your Cat's Health History

Cats are considered geriatric by the time they are 10 years old. Older cats may need to have a record of a vet exam within 12 months prior to being groomed. Certain medical conditions make grooming a cat extremely hazardous and potentially fatal. Statement of Health and Release

Please certify the following by checking each box.

I confirm that my cat is in good health and is free of external parasites and contagious diseases. l will not hold Downtown Tabby Inc. responsible for any pre-existing health problems my cat may have. Any grooming which takes place on an elderly animal or animal with health problems will be at my own risk. Grooming may expose pre-existing health/skin problems for which Downtown Tabby Inc. cannot be held liable.

I understand that de-matting/thinning out of matted hair may cause patchiness and hair loss. While Downtown Tabby Inc. makes every effort to comb out mats, Downtown Tabby Inc. may feel that this is too painful for my cat. In such case, Downtown Tabby Inc. may request my permission for shaving.

I understand that shaving/stripping my cat will dramatically change it’s appearance, and the hair will be very close to the skin. This may expose pre-existing skin issues.

I understand that Downtown Tabby Inc. is not liable for veterinary bills, not caused by gross negligence, that may be incurred in the procedure of the groom or for any pre-existing growths that are cut or irritated during the grooming process. I agree to advise the groomer of any pre-existing blemishes that might be injured during the grooming process to avoid any problems.

I agree not to hold Downtown Tabby Inc. responsible for any adverse reaction or occurrence as a result of a grooming procedure.

In the event of any injury or health concern, Downtown Tabby Inc. will attempt to contact me at the primary number listed which I provided. They will also try to contact the emergency number provided by me. If we cannot be reached, and an occurrence is determined to be an emergency, I understand that Downtown Tabby Inc. will contact a veterinarian, and I authorize any necessary treatment for the cat. I agree to be responsible for any and all veterinary fees that may result. I will not hold Downtown Tabby Inc. liable for any damages, including loss of life of the animal.

I certify, to the best of my knowledge, that my cat has not been exposed to ringworm, rabies, distemper or any other infectious diseases; I agree to provide a current RABIES & DISTEMPER CERTIFICATE (and/or health certificate) before service, in addition to vaccination records, for outdoor cats.

I agree to pay for services provided by Downtown Tabby Inc. for any and all grooming procedures. Owner will be responsible for payment of any services that are completed as agreed upon. Payment is due upon the completion of groom as per price list/quote. Cancellations with less than 24 hours advance notice, except in cases of emergency, will require pre-payment when scheduling next visit.

This contract contains the entire agreement between the parties. All terms and conditions shall be binding on the heirs, administrators, person representatives and assigns of the owner and Downtown Tabby Inc.

Any controversy or claim arising out of or relating to this contract, or breach thereof, or as the result of any claim or controversy involving the alleged negligence by any party to this contract, shall be settled in accordance with the rules of the American Arbitration Association. Judgment upon the award rendered by an arbitrator may be entered in any court having jurisdiction thereof. The arbitrator shall, as part of the award, determine an award to the prevailing party of the cost of such arbitration and responsible attorney’s fee of prevailing party.
